DOT Medical Card for Truck Drivers: How Long It Lasts
A DOT medical card is valid up to 24 months. Learn who can issue it, how Texas tracks it and what happens if it expires. This is not medical advice.

Your DOT medical card is easy to forget until the day it expires. Then it can stop you from driving. Here is what every truck driver should know about the card, in plain words. This is general information and not medical advice. Talk to a medical examiner about your own health.
What a DOT medical card is
Your DOT medical card is a short form called the Medical Examiner's Certificate. It shows that a certified medical examiner checked you and found you physically able to drive a commercial truck. If you drive a commercial motor vehicle across state lines, you generally need one. It is also part of your driver qualification file. Our article on the driver qualification file explains that file.

A DOT medical certificate is valid for up to 24 months. That is two years. The examiner may give you a shorter time, such as one year, when a health condition needs to be watched. For example, that can happen with something like high blood pressure. A shorter card is not a failure. It just means the examiner wants to see you sooner.
Who can issue it
Not just any doctor can give you a valid card. The exam has to come from a medical examiner who is listed on the FMCSA National Registry of Certified Medical Examiners. FMCSA is the Federal Motor Carrier Safety Administration. The examiner has to be on that list on the day of your exam. If you go to someone who is not listed, your card may not count.
Before you book, ask the clinic: "Are you on the FMCSA National Registry?" You can also look up an examiner on the National Registry website.
How Texas tracks your medical card
If you have a Texas CDL, the Texas Department of Public Safety, or DPS, is also part of the process. Here is what DPS says:
- Texas asks CDL holders to certify to one of four categories. Most drivers who run across state lines fall in the non-excepted interstate group, which needs a medical examiner's certificate.
- Since June 23, 2025, certified medical examiners send your exam results to DPS electronically. They have to send them by midnight the next calendar day after the exam.
- Allow up to 10 business days for the update to show up on your record.
- DPS sends a notice about 60 days before your certificate expires.
What happens if it expires
If your medical certificate expires, your Texas CDL can be downgraded, and you lose your commercial driving privileges. In plain words, you are no longer certified to drive a commercial truck. Driving anyway can bring big trouble, and you may not be able to haul a load until it is fixed.
Do not count only on the DPS notice. Mail can get lost, and life gets busy.
How to stay ahead
- Set two reminders. One for 90 days before expiration and one for 60.
- Book your exam early. You want time to fix a problem if the exam does not go as planned.
- Bring what you need. Bring a list of your medications and your doctor's information.
- Make a copy. Keep a copy in your driver qualification file and one with your truck papers.
- Check that the update posted. After your exam, confirm your record shows the new card.
Mistakes to avoid
- Waiting until the last week.
- Going to an examiner who is not on the National Registry.
- Losing track of the date after you renew.
- Not putting the copy in your file. An auditor may ask for it.
